
Changes to the Register
Change of Address
If you are a registrant, you may notify the Board by letter or telephone of your address change. You should state your previous and new address. If the new address is your residential address, you should state whether or not you wish that address to appear on the Register of Physiotherapists which is available to the public. If not, you may indicate an alternate address for Public Register purposes.
Notification by phone is available during business hours and will require you to also provide details to confirm your identity to enable the change to be effected.
Once the Register address has been amended, you will receive a confirmation letter advising that the Board's records have been updated.
Change of Name
Registrants must submit the following:
- a written request for a change of name; and
- a certified copy of marriage certificate / deed poll document / decree nisi.
Duplicate Certificate of Registration
If the original Certificate of Registration has been lost, destroyed or damaged, a Statutory Declaration stating same is required. In addition, it will be necessary to submit the prescribed fee of AUD $28.00. If a change of name is required to be noted on the Original Certificate, the Certificate should be returned with the prescribed fee and certified documentary evidence of change of name (see Change of Name).
Notification of Business Names
Please refer to sections 126 and 127 of the Physiotherapists Registration Act 2001..
